What Causes Worn Teeth and Options to Fix This

What happened to these worn teeth?  Were these damaged from grinding or TMJ disorders?  Are these teeth short or small? Were they worn down by acidty? What is causing this?

Does the patient have any other issues from TMJ, neck or facial pain, ringing of the ears,headaches, or sleep apnea? Is this the result of a misaligned bite?
