  The purpose of this editorial is to give some insight to some of the complex cosmetic dentistry cases that we see. The before photo is a picture of a 38 year old who came to see us for a cosmetic dentistry consultation.  Little did he realize that he had TMJ problems and bite problems that were ruining his teeth.  There are two ways to approach this case: 1) Do conventional cosmetic dentistry with porcelain veneers 2)  Treat the patient with neuromusuclar dentistry and TMD bite management, then do a smile makeover If option one was chosen, what would keep the patient from ruining the new porcelain veneers as conventional dentistry would have restored the teeth to the same bite position that he ruined his natural teeth in. Option two, using neuromuscular dentistry and advanced TMJ treatment, will allow us to find the optimal and stable bite position for his jaw joints, muscles, and teeth. After this is ascertained and we know that the patient is comfortable, the restorative care and veneers can be designed to this new corrected bite position so that the dental care will last a long time.  Overall, this patient was elasted with the results and loves the comfortable feeling of having a stable bite.
