How do you adjust dental sleep appliances?

Dear Dr. Konig:

I have sleep apnea (AHI is 25.9, but in supine sleep it is 39.1). Cannot tolerate CPAP-severe stomach distention. Considering an oral appliance, such as Somno Med MAS. Have some concern about not being able to titrate it in a sleep study to know exactly where it needs to be for optimum air flow. Thanks.



You are not alone with CPAP Intolerance! The Somnomed is a nice appliance and is one of the ones we use to assist our patients. You ask a very good question with respect to titration.
After we take the measurements for the appliance, we deliver the appliance and allow you to sleep with it for one week, after this week we provide a home montior which will tell us the AHI, RI, as well as the Oxygen saturation level and more. From this Dr. Konig will titrate the appliance as indicated. This is repeated several times over a month or two, depending on the patient.

This protocol allows use to adjust your appliance for maximum airflow. Without the use the home monitors there is no way to titrate as the sleep lab cannot wake you up every hour to adjust this. Many times our patients return to the sleep lab after we complete all titrations for a final PSG. The home monitors are, however, very accurate. It is essential that appliances are titrated properly.
